A case of severe infection of biliary tract.
On coronal plane, in no doubt, we all certainly see a stone as if explicitly caused the dilatation of CBD. The stone’s nearly 6mm at the end of cbd actually. Unfortunately, on ultrasound scanning, my colleagues couldn’t see the stone at all because of thicken wall of her abdomen. So that the problem is almost all sonographers should make same mistake even though they carefully scan fat patients with ultrasound indeed. However, it is generally accepted that we need to check these patients with ct scanner instead. As you can see, in this case, an ct scan was used to examine the patient lately.
In my opinion, the correct diagnosis was a little bit late in this case (delayed diagnosis). Therefore she was not getting a little bit better after several days of treatment. Doctors couldn’t help her any longer. As usual, She has been transfered to other hospital as a result of severe infection of biliary tract resulted in infective shock inevitably. One argument puts forward is that she need to be performed an ERCP which will be the best choice to take the stone away immediately.
