I has a new patient, who has suffered from an acute abdominal pain in his right upper quadrant.He also has a slight fever for several days before coming to hospital. In this video, we could see the complete dilatation of biliary duct, particularly dilated in CBD that we measured nearly 28 mm in diameter with gas inside the biliary tract. Let’s have a closer look at the terminal intraduodenal portion of the common bile duct in which, having a stone with light shadow caused narrowing of the CBD, i supposed. However, in my experience this stone could not be a cause of the obstruction in this case at all. I believe that the first diagnosis must be congenital cyst (type 1) in CBD, therefore the acquired dilatation of CBD should be a different diagnosis.
It is generally accepted that Type 1 choledochal cysts 80% to 90% of cases) are focal, saccular or fusiform, dilatations of the common bile duct that was most likely to look like our case. How about your opinion? Please drop a line here. Thank so much.
